Insurance Information MEDICAL BENEFITSAs a Los Lunas Schools employee you get to choose between two carriers for your medical insurance:Blue Cross/Blue Shield High Option/ Low Option or Presbyterian High Option/Low Option What should drive your choice between the two carriers is the network of doctors that you prefer to belong to. The plans are the same for both carriers. The following is a list of co-payments you can expect if you choose the “High Option Medical Plan”: Office Visit $20.00 per visit MRI or PET scans 200.00 per day CT scans 50.00 per day Urgent care 50.00 copay Emergency room 100.00 copay Hospital admission 500.00 copay The “Low Option Medical Plan” is a catastrophic plan. The calendar year deductible for the low option plan is $1,500.00 for single coverage, $3,000.00 for 2-party coverage or $4,500.00 for family coverage. You are allowed doctor’s visits for a co pay of $20.00 per office visit. DENTAL BENEFITSLos Lunas Schools dental insurance is through United Concordia and offers both a High Option Plan and a Low Option Plan. The difference between the two plans is that the High Option plan covers the full range of dental services while the Low Option plan offers basic services only. There is a $50.00 calendar year deductible for each family member up to $150.00. Although you may use a dentist that is outside the United Concordia network, you will get the best benefit from the plan if you stay within their network of dentists.PRESCRIPTION BENEFITSPrescriptions are through Cataylst Rx. You will pay $2.00 for a Generic, 30% of the cost of the Rx for a name brand and 40% for a non-formulary.VISION BENEFITSLos Lunas Schools vision insurance carrier is Vision Service Plan. The plan allows you to get an examination and lenses once a year, frames every two years and contacts in lieu of glasses. There is a $15.00 copayment for the exam and a $15.00 copayment for the lenses. They cover single, bifocal or trifocal lenses and some of the special coatings such as scratch resistant or "anti glare".LIFE INSURANCEIs a Los Lunas Schools Employee you receive basic life insurance through The Standard in the amount of $25,000. This is free of charge to you. If this is not enough to meet your needs you can purchase more life insurance at one, two or three times your salary. If you purchase more on yourself, you can also cover your spouse and child(ren).LONG TERM DISABILITY INSURANCELong term disability insurance is provided through The Standard. Our plan picks up on the 31st day and pays 66 2/3% of your salary. If you want to participate in our sick bank, you must purchase this insurance.Back to Personnel
